The natural instinct is to look at a no-show and think: lost treatment fee. Someone was booked for a $250 filler appointment and didn't come in. That's $250 gone.
That's true. It's also only the beginning.
Here are the less obvious losses at play every time a patient doesn't show:
- The retail purchase didn't happen. Most patients who complete appointments also buy product at checkout. It's one of the highest-margin moments in your practice, and it happens naturally as part of the visit flow. Your provider recommends a post-treatment product, the front desk follows up at checkout, and that conversation has real dollar value attached to it. When the appointment doesn't happen, neither does that revenue.
- The rebooking never took place. A completed visit almost always ends with a next appointment. A no-show breaks that rhythm entirely. No follow-up booked, no future revenue secured. That patient just became someone you now have to actively re-acquire, which costs more time and money than simply retaining them would have.
- And if they don't come back? A patient who goes uncontacted after a missed appointment often doesn't come back at all. At that point you're not calculating for one missed slot. You're accounting for the full lifetime value of a patient who quietly walked out the door.
- The provider's time is gone. Twice. That hour was booked, reserving a block of paid time in their schedule. The second is what that hour could have been. A provider's schedule only has so many bookable hours in a day, and a no-show means one of them went to nobody. You have to consider what you paid out, and what you never got to earn. A debit with no credit.
Add all of that together — sunk labor cost, missed retail, lost rebooking, patient churn — and the real cost of a no-show is meaningfully higher than the treatment fee.
A Better No-Show Formula
Just three numbers can get you a clearer picture:
- Step 1: Find your no-show rate. Take the appointments that were scheduled but never completed. Not cancellations rescheduled with enough notice to fill the slot, but true no-shows and last-minute drops you couldn't recover. Divide by your total scheduled appointments for the month and multiply by 100.
No-show rate = missed appointments ÷ total scheduled appointments × 100
- Step 2: Calculate your direct revenue loss. Multiply your monthly no-shows by your average Revenue Per Visit. This is the number most practices stop at.
Direct loss = no-shows × average RPV
- Step 3: Add what most practices miss. What's your average retail purchase at checkout? Multiply that by your no-show count and add it to Step 2. That's your real monthly loss.

Turning the Numbers Into Action
You recognized no-shows are a problem, calculated yours, now how can you make a change? Here are a few levers that have an outsized impact on recovery.
- Require deposits for high-ticket services. A deposit doesn't just protect your revenue, it also changes a patient's relationship to the appointment. They are invested from a time and money standpoint before entering the door. Skin resurfacing, body contouring, and other premium treatments are natural candidates.
- Implement a clear cancellation policy. And enforce it consistently. Practices that post their policy at booking, in their confirmation messages, and at the front desk see meaningfully better outcomes. Flexibility for genuine emergencies is appropriate; inconsistency is not.
